Start with the contact form you actually need
For a straightforward message, Shopify’s built-in contact page is a sensible starting point. Create a page in Online Store → Pages, give it a title, choose the contact theme template and save. Add it to your store navigation so customers can find it. Submissions go to your store’s sender email address.
Shopify documents that process in its contact page guide. Some themes also let you add a contact form section to another page.
Choose a custom form when you want to arrange fields visually, collect a file alongside the message, keep a searchable response history or connect the enquiry to a workflow. Perfect Forms by Merle Studio supports those jobs from inside Shopify.
Ask enough to make the first reply useful
A general contact form does not need to double as a customer survey. Make the reply address and message required. An order number is helpful for an existing purchase, but a prospective customer will not have one.
| Field | Type | Why ask? |
|---|---|---|
| Name | Short text | Know how to address the customer. |
| Send a reply. Make this required. | ||
| What can we help with? | Dropdown | Separate product questions, order help and other enquiries. |
| Order number | Short text | Optional context for an existing order; keep symbols and leading zeros. |
| Message | Long text | Let the customer explain the request in their own words. |
| Photo or document | File upload | Optional evidence when a picture explains the issue faster. |
Use a short text field for order numbers rather than a numeric field: order references can contain letters or a leading #. Label optional fields clearly. A customer should be able to ask a pre-purchase question without inventing an order number.
Build and place the form with Perfect Forms
- Install Perfect Forms, open Forms → Create form, and start with the contact fields. Add a dropdown or file upload if your support process needs one.
- Use Heading and Body elements to explain what the form is for. Keep the visible heading friendly and the admin title specific, such as “Main contact page”.
- In Layout → Presentation, choose an inline layout. Adjust fonts, colours and spacing in Styles. Check the Desktop and Mobile previews.
- Save and Publish. On the form’s detail page, choose Add to theme, copy the Form ID and put it into the Perfect Forms Form app block. Your chosen theme section must support app blocks.
- Save the theme and submit a test from the actual contact page. Open View responses to confirm it arrived.
Publishing makes the form available; placing the Form block makes it appear on your store. If you replace the theme’s contact form, remove the old contact section from that template so visitors do not see two competing forms.
Tell customers what happens next
Put a realistic reply window above the form. “Our team replies Monday to Friday” is more useful than a promise of an instant answer you cannot keep. Include a link to delivery or returns information if it answers a common question.
For alerts, set a recipient in Settings → Email notifications and enable Email notifications on this form. Alerts link your team to the response in the app. They do not include the customer’s answers or attachments in the email.
Keep marketing choices separate from a request for support. Someone asking where their order is should understand exactly what they are submitting and why you need their details.
Run one complete test before sharing it
- Open the contact page on a phone and complete it with the onscreen keyboard.
- Try an empty required field and an invalid email address. Check that you can understand and fix the error.
- Send an enquiry without an order number, then another with an attachment if uploads are enabled.
- Check the saved response, any uploaded file and the notification recipient.
- Follow the contact link from your store’s navigation, not only from the theme editor.
After the first few real enquiries, look for repeated follow-up questions. Add a field only if it would remove that extra round of conversation.
