Put the essentials above the application
Use a dedicated wholesale page. Explain the kind of retailers you work with, the regions you serve and any opening-order requirements you are ready to share. A shop owner should be able to judge whether an application makes sense before filling it in.
You can use this as a starting point for your introduction:
Interested in stocking our products? Tell us a little about your shop, where you sell and the collection you have in mind. Our team will review your application and contact you about the next steps.
Add your actual review window if you have one. Avoid promising acceptance or access to trade pricing before someone has reviewed the business.
A field list for your first wholesale form
| Field | Type | Why ask? |
|---|---|---|
| Business name | Short text | Identify the prospective retailer. |
| Contact name | Short text | Know who is managing the application. |
| Business email | Keep the application and follow-up together. | |
| Website or shop profile | Short text | Understand their range, audience and presentation. |
| Country or region | Dropdown | Check whether you can serve the retailer. |
| Where do you sell? | Dropdown | Offer physical shop, online shop, both, and other. |
| Products of interest | Long text | Find out which collections they want to stock. |
| Expected opening order | Short text | An optional estimate to help plan a conversation. |
If supporting documents are essential to your review, add a File upload field and explain exactly what you need. Otherwise, ask for them later. Gathering every possible detail upfront makes the first step harder without necessarily improving your decision.
Do not use a number field for a business registration reference: identifiers can contain letters and leading zeros. Likewise, a business email is a contact detail, not proof that an applicant represents the business.
Build the application in Perfect Forms
In Perfect Forms, create a form and give it an admin name such as “Wholesale applications”. Replace the starting contact fields with your application fields, then add a Heading and Body to introduce the request.
Choose an inline presentation in Layout. A dedicated page gives applicants room to read your wholesale information and complete a longer form without a popup interrupting them. Match your store’s typography, colours and spacing in Styles.
Label the submit button “Send application”. Save and publish, then use Add to theme to place a Perfect Forms Form block on the wholesale page template. Enter the Form ID, save the theme and link the page from your navigation or footer.
Check which pages use that template. If the template is shared, the app block may appear on other pages as well. Give the wholesale page its own template when you want the application in only one place.
Give every application a next step
Choose who reviews new applications and how often. Enable email notifications for this form, or use the Perfect Forms trigger in Shopify Flow to route a notification through a workflow.
- Review the retailer’s location, shop and product interests.
- Ask for any missing information needed to make a decision.
- Send the appropriate next step: a conversation, your wholesale information, or a clear explanation that it is not a fit.
- Record the decision in the system your team uses to manage wholesale relationships.
Perfect Forms collects the application. It does not automatically approve a Shopify B2B company, assign a catalogue, grant trade pricing or create a customer account. Configure any subsequent process separately and check the Shopify features available to your store.
Review it as if you were a stockist
Complete the form for an online-only shop and for a physical retailer. Can both answer every required question honestly? Can a shop owner explain an unusual situation without being forced into the wrong dropdown option?
Submit a test application from your phone. Check that your team can find the response, follow the website link and understand the applicant’s interests without sending the same questions again. That is a more useful measure of the form than how many fields it contains.
