A form that feels at home in your store
You put care into your products, your photography and the way your store feels. The form at the end of the journey deserves some of that attention, too.
Perfect Forms gives you a live canvas to work on. Add fields, move them around and adjust fonts, colours, spacing and images. Switch between desktop and mobile previews while you build. There’s also custom CSS for the details you want to fine-tune.
The starting point is a contact form, but the questions are yours. You can collect a product enquiry, a wholesale application, feedback or the details for a custom project. Short text, long text, email, number, date, dropdown, checkbox and file upload fields give you the pieces to put it together.
Shopify Sidekick can help build the form as well. You can then check the result in the visual editor and make it your own before publishing.
Some requests need more than a message
A customer explaining a damaged item might need to send a photo. A prospective stockist might want to share a document. Someone asking about a custom print might already have the artwork ready.
File uploads keep those details together with the request. Your team can open a response in Perfect Forms, read the answers and download the attachments from there. You can also search and filter responses and export them to CSV.
For a form that belongs on a particular page, use an inline layout. For a shorter invitation, choose a popup or a floating button that visitors can open themselves. The placement guide explains the options and what to check on a phone.
Give the answer somewhere useful to go
Collecting a response is one step. Getting it to the right person is the next.
Set up email notifications for the forms your team needs to watch. If your process needs more than an alert, the Perfect Forms Form response submitted trigger connects submissions to Shopify Flow. Filter by form, check an answer and use the actions available in your store’s workflow.
Start with one simple handoff. A wholesale application might go to the person managing stockists; a product enquiry might go to support. Our Flow guide walks through the trigger, answer fields and the checks that keep a workflow useful.
A small place to start
The free plan includes the visual builder, custom styles, file uploads and up to 100 responses per month, with a Perfect Forms watermark. Paid plans offer more responses and remove the watermark. You can compare the current options on the pricing section.
If you’re trying it for the first time, pick one useful form. Build it, place it in your theme, submit a test and read the response as the person who will handle it. That complete journey tells you more than a preview alone.
